1. January-Receipt of rate letter for January, February and March, using the CMI from the October 1 point-in-time date.
2. February 1-Cutoff date for acceptance of MDS transmissions for inclusion in preliminary CMI listing for the point-in-time date of January 1.
3. February 15-Myers & Stauffer mails out the preliminary CMI listing for the January 1 point-in-time date. 
4. March 1-Cutoff date for acceptance of MDS transmissions for inclusion in the final CMI listing report for the January 1 point-in-time date.
5. March 15-Myers & Stauffer mails out the final CMI listing for the January 1 point-in-time date.
6. April-Receipt of rate letter for April, May and June, using the CMI from the January 1 point-in-time date.
7. May 1-Cutoff date for acceptance of MDS transmissions for inclusion in preliminary CMI listing for the point-in-time date of April 1.
8. May 15-Myers & Stauffer mails out the preliminary CMI listing for the April 1 point-in-time date.
9. June 1-Cutoff date for acceptance of MDS transmissions for inclusion in the final CMI listing report for the April 1 point-in-time date.
10. June 15-Myers & Stauffer mails out the final CMI listing for the April 1 point-in-time date.
11. July-Receipt of rate letter for July, August and September, using the CMI from the April 1 point-in-time date.
12. August 1-Cutoff date for acceptance of MDS transmissions for inclusion in preliminary CMI listing for the point-in-time date of July 1.
13. August 15-Myers & Stauffer mails out the preliminary CMI listing for the July 1 point-in-time date.
14. September 1-Cutoff date for acceptance of MDS transmissions for inclusion in the final CMI listing report for the July 1 point-in-time date.
15. September 15-Myers & Stauffer mails out the final CMI listing for the July 1 point-in-time date.
16. October-Receipt of rate letter for October, November and December, using the CMI from the July 1 point-in-time date.
17. November 1-Cutoff date for acceptance of MDS transmissions for inclusion in preliminary CMI listing for the point-in-time date of October 1.
18. August 15-Myers & Stauffer mails out the preliminary CMI listing for the October 1 point-in-time date.
19. December 1-Cutoff date for acceptance of MDS transmissions for inclusion in the final CMI listing report for the October 1 point-in-time date.
20. December 15-Myers & Stauffer mails out the final CMI listing for the October 1 point-in-time date.
